How to Apply
When you eapply for Ohio Medicaid online, you can choose Buckeye Health Plan as your health plan. You will have an option to select your Primary Care Provider (PCP) from the Buckeye Provider Directory. If you need to change your PCP you can call Member Services to ask for a change. Members are permitted to change PCPs monthly.
Three ways to check your eligibility and apply:
- ONLINE: Go to the Ohio Benefits website — this is the fastest way!
- PHONE: Call the Ohio Medicaid Consumer Hotline at 1-800-324-8680
- IN PERSON: Visit your County Department of Job and Family Services office. Find your county office at the JFS website
Enrolling and Choosing Buckeye Health Plan is Easy
Buckeye Health Plan offers Ohio Medicaid health insurance for children, pregnant women, families with children, elderly, and adults and children with disabilities.
If you think you might be eligible for Ohio Medicaid, you can apply for Ohio Medicaid with Ohio Benefits. When you apply for Ohio Medicaid, you can choose Buckeye Health Plan as your health plan.
You can apply for Medicaid services and enroll in a health plan provided by Buckeye Health Plan online:
- Apply for Ohio Medicaid at the Ohio Benfeits Portal
- Choose Buckeye Health Plan as your health insurance provider
- Then you will receive our welcome packet
- Enjoy Buckeye Health Plan Ohio Medicaid benefits, at no cost to you
